Angiogram
adalah satu kaedah atau ujikaji pengimejan yang menggunakan X-Ray bagi
melihat saluran darah dalam badan kita samada urat-urat darah itu
sempit, tersumbat, membesar atau sebagainya di beberapa bahagian badan
termasuklah otak kita, jantung, abdomen dan kaki.
X-ray
equipment is mounted on a C-shaped gantry with the x-ray tube itself
beneath the table on which the patient lies. Above the patient is an
image intensifier that receives the x-ray signals, amplifies them, and
sends them to a TV monitor.
Untuk
menghasilkan imej X-Ray, pakar akan memasukkan (inject) sejenis cecair
atau kadang kala dipanggil "dye" melalui tiub halus yang flexible
dikenali sebagai "catherer". Dye ini atau lebih tepat dipanggil
"contrast" akan menyebabkan aliran darah yang mengalir di dalam urat
darah dapat dilihat ketika X-Ray. Pakar akan membuat bukaan untuk
memasukkan catherer pada bahagian pangkal peha atau melalui tangan
(bergantung kepada situasi dan keadaan pesakit). Tiub halus catherer ini
akan melalui saluran darah hingga ke tempat tuju i.e. jantung dan
sebagainya.
Kadang-kadang doktor pakar boleh menangani beberapa masalah urat saraf
ketika angiogram. Contohnya, merungkai darah beku (clot) yang ditemui
semasa ujikaji, atau melaksanakan kaedah angioplasty dan stenting pada
urat saraf yang tersumbat bergantung kepada lokasi dan keadaan.
Angiogram juga dapat membantu pakar merancang pembedahan untuk membaiki
urat saraf yang berkeadaan lebih rumit serta bermasalah.

The
heart is a muscular organ about the size of a fist, located just behind
and slightly left of the breastbone. The heart pumps blood through the
network of arteries and veins called the cardiovascular system.
The heart has four chambers:
- The right atrium receives blood from the veins and pumps it to the right ventricle.
- The right ventricle receives blood from the right atrium and pumps it to the lungs, where it is loaded with oxygen.
- The left atrium receives oxygenated blood from the lungs and pumps it to the left ventricle.
- The left ventricle (the strongest chamber) pumps oxygen-rich blood to the rest of the body. The left ventricle’s vigorous contractions create our blood pressure.
The
coronary arteries run along the surface of the heart and provide
oxygen-rich blood to the heart muscle. A web of nerve tissue also runs
through the heart, conducting the complex signals that govern
contraction and relaxation. Surrounding the heart is a sac called the
pericardium.
Nota:
For angiography, a thin, soft tube (catheter) is placed in a blood
vessel in the groin (such as in the femoral vein). First, a doctor numbs
the area with a local anesthetic. Then, a needle is placed into the
blood vessel. A guide wire is put through the needle into the blood
vessel and the needle is removed. The catheter is slid over the guide
wire and moved into the blood vessel. The catheter is guided through the
blood vessels until the tip is in the area to be studied.

Hasil
dari Kaedah Angiogram boleh direkod dan disimpan untuk kajian lanjut
pakar. Di bawah adalah foto X-Ray saya selepas menjalani Angiogram kedua
pada 23 Jun 2010 di Hospital Sultanah Bahiyah, Alor Setar, Kedah (Dr.
Hj. Abdul Rahim bin Tahir - Ketua Jabatan Kardiologi).
Jika terdapat saluran darah yang tersumbat, doktor pakar akan membuat keputusan samada memasukkan "stent" atau spring ataupun kaedah belon untuk membuka saluran tersebut.
Many angioplasty procedures also include the placement of a stent, a small, flexible tube made of plastic or wire mesh to support the damaged artery walls. Stents can be self-expandable (opens up itself upon deployment) or balloon expandable (balloon needed to open the stent). Balloon expandable stents are typically placed over a balloon-tipped catheter so that when the balloon is expanded, it pushes the stent in place against the artery wall. When the balloon is deflated and removed, the stent remains permanently in place, acting like a scaffold for the artery. Self-expandable stents are easy to deploy, but may require additional angioplasty with balloon to obtain satisfactory dilation (opening) of the diseased vessel. Covered stents or stent-grafts have additional advantages over bare stents and are becoming more commonly used.

Nota: pemasangan stent pada urat atau saluran darah adalah kekal dan tidak boleh ditanggalkan i.e. pakai sampai mati. Manakala belon ialah sementara i.e. dikembangkan belon pada saraf yang tersumbat (biasanya urat saraf yang terlalu halus dan tidak dapat dipasang stent), dibuka saraf tersebut, dikincupkan belon dan dikeluarkan.
